Our Exciting Wellness Fair will be held on February 18, 2025!
As a part of the program, you can register NOW for your 3-D Mammogram and become eligible to receive a $25 gift card.
You have two convenient ways to schedule your appointment:

- Everyone who registers and completes their exam will receive a $25 gift card!
- All insurance plans are accepted. If you don鈥檛 have insurance, self-pay is available for $200.
- Bring your insurance card and photo ID to your appointment.
- Women must be age 40 or older to qualify (or age 35-39 for a baseline mammogram).
- Mammograms are covered (as preventative) 100% by insurance once every 12 months.
- Appointments are available on Tuesday, February 18, from 9 am to 2:45 pm.
- Upon registering for an appointment, be prepared to provide:
- Your Primary Care Physician (PCP) or OBGYN’s name & address and
- The facility name and date of your last mammogram.
